The following are provisions of mandated requirements for all Clairton City School District employees. Foremost, there are three (3) clearances required by the Pennsylvania Department of Education in order for a worker or volunteer at a school district in any capacity including, but not limited to, teachers, paraprofessionals, substitute teachers, office staff, maintenance, cafeteria staff, security, bus drivers, club sponsors and coaches.  Those clearances are as follows: Act 34, Act 114 and Act 151.

  • ACT 24 – Arrest/Conviction Report and Certification FormAvailable from the PDE web site by clicking here
  • ACT 34 – Pennsylvania Criminal History Check -
    COST: $22.00

    Go to “New Record” and follow the instructions through to the Certification Page. Once the Search Results Table appears, click on the Control Number. (Hint: Write down the Control Number for future reference.) The Record Check Details page is opened. Click on the Certification Page to access your official Clearance. The Record Check Details page is only a receipt and not acceptable as a ‘clearance’. Print 2 Certification Pages, 1 for you and one for your Personnel File.

  • ACT 114 – FBI Clearance – To schedule your ten-minute fingerprint appointment, simply visit and enter the following service code 1KG6XN. This Service Code is unique to your hiring/licensing agency.  Do not use this code for any other purpose.
    COST: $23.85

    Register with IdentoGO through the above site (available 24 hrs/day). Make sure you register for the Department of Education FBI clearance.  Take your receipt and a valid photo id to your enrollment appointment.

  • ACT 151 – Pennsylvania Child Abuse Clearance
    COST: $13.00

  • ACT 126 - Mandated Reporter
  • ACT 168 – Sexual Misconduct/Abuse Disclosure Release Act 168 Form
    This circular describes the process for completing the employment history review process related to Act 168, explains the use of forms for employment screening, and details the process for accessing "pending criminal charge" information as required by Act 168.
  • Please email to or mail via US Postal the original form(s) with the Name of Current or Former Employer(s) and Section 1 completed. DO NOT MAIL TO FORMER EMPLOYER. THEY ARE SENT BY THE DISTRICT.
  • The Clairton City School District requires that every employee be drug tested. The cost of this test will be paid by the district.  All of the above forms and information can be obtained at the Administration Building, 2nd floor.
